All billing and fees are managed through our Office Manager.

On Fees & Billing

 

As a courtesy FSAC will Contact your insurance for preauthorization, continued authorization and bill for all appropriate services. Our Office Manager will work to get your benefits approved, however, please remember you are the best advocate for your benefits and how they are paid.

 

Copay's are due weekly and a statement will be sent to you each month. Should you encounter any difficulty keeping up with CoPays, please contact our Office Manager at the main office as soon as possible so that we may make other arrangements with you.
